OSHA fines Greenheck for employee death in July - WSAW

SCHOFIELD, Wis. (WSAW) - OSHA has fined Greenheck Group after closing its investigation into an employee death back in July.

Longtime Greenheck employee Frank Lotharius died after being injured while operating an Integrated Blanking System which is a combination of four machines including a punch press, a laser cutting table, a sheet master, and a STOPA sheet metal tower.

According to OSHA documents, around noon on July 10, Lotharius was observing the IBS completing a production run. During the run, a metal sheet had become dislodged from a cart and caused a fault in the sheet master. Lotharius noticed the problem and went to correct it by grabbing the operational key for the punch press and entering the east side door of the IBS, which has an interlock connected to the punch press and sheet master. Lotharius then took a crowbar and began to pry the sheet back into place on the cart. At the same time, another cart had been called to travel into the same area to collect a completed metal cut from the laser. When the other cart traveled forward, Lotharius was caught between the top of the first cart and the underside of the second cart.

Greenheck has been fined $31,250 by OSHA for violations related to the incident.
